Before you add photos, please review these important guidelines to protect your patients, clients, and practice.
⚠ Photo privacy requirements
Photos used in envizit narratives must never contain identifiable individuals without their written consent. This includes:
Patients or clients in any stage of treatment
Staff members who have not signed a release
Any person visible in the background of a photo
All photos should be taken during non-patient hours, or use consented models only.
Model and photo release forms
If anyone appears in your photos, they must sign a release form first. Download the appropriate form below and keep signed copies on file.
About HIPAA and envizit
envizit narratives describe visit experiences in general terms and do not store patient names, diagnoses, or treatment records. Photos you upload are stored securely and used only to display your narrative.
If your practice is a HIPAA-covered entity, you are responsible for ensuring your use of envizit and the photos you upload comply with your obligations under HIPAA. envizit does not store protected health information (PHI).
I understand that all photos must be taken with appropriate consent, during non-patient hours or with consented models, and must not contain identifiable individuals without a signed release on file.
